• The HealthTalent Nurse Referral form below must be filled out completely by the you and submitted to our business office prior to the referred nurse’s contact with HealthTalent for employment.
• A Nurse Recruiter will follow-up on the referral by contacting the candidate and, if applicable, processing the candidate’s employment application. If a referred nurse does not complete the application process and begin employment with HealthTalent within 90 days from the date he or she is referred, you will not receive a referral fee and/or commission bonus.  In the event that there is a dispute about who referred a candidate, we will resolve the dispute directly with the candidate referred.  If we are unable to reach a determination, no referral fee or commission will be paid.
• If you are a current full-time employee of HealthTalent you will be eligible for a commission for each hour billed and worked by each referred nurse through the duration of your assignment and up to 30 days after. If you start another assignment with HealthTalent in less than 30 days after the completion of your previous assignment, you will continue to receive commissions on each hour billed and worked by each referred nurse. If you wait longer than 30 days to start another assignment, you will no longer receive commissions on those nurses you have referred.
