•    The HealthTalent Nurse Referral form below must be filled out completely by you and submitted to our business office prior to the referred nurse’s contact with HealthTalent for employment.

•    A Nurse Placement Specialist will follow-up on the referral by contacting the candidate and, if applicable, processing the candidate’s employment application. If a referred nurse does not complete the application process and begin employment with HealthTalent within 90 days from the date he or she is referred, you will not receive a referral fee and/or commission bonus.  In the event that there is a dispute about who referred a candidate, we will resolve the dispute directly with the candidate referred.  If we are unable to reach a determination, no referral fee or commission will be paid.

•    After the referred candidate becomes an employee of HealthTalent and works one 13-week assignment (or the equivalent of), a referral fee of $1,000 will be paid directly to you.  The travel nurse must successfully complete the entire assignment for eligibility.
